Increased risk of incident atrial fibrillation in young adults with mental health disorders: a nationwide population-based study

Abstract Background Mental health disorders and cardiovascular diseases are closely associated. The impact of mental on disease risk could be related to associated smoking, high blood pressure, cholesterol. Despite shared factors, there remains a paucity information incident atrial fibrillation (AF) incidence in patients with disorders. Purpose To assess the association between AF, especially amongst young adults aged 20 39 years. Methods Using Korean National Health Insurance database 2009 2012, we identified years without history AF who have been diagnosed diseases. were defined as having one following diagnoses: schizophrenia (ICM-10 code F20), bipolar disorder codes F30, F31), depression F32, F33), insomnia G470, F510), anxiety F40, F41). primary outcome was new-onset during follow-up. We compared those without. Results A total 6,576,582 people ages 20–39 old (mean age, 30.9±5.0 years; male, 59.6%) included this study. Among population, 668,683 (10.2%; mean 31.4±5.0 female, 53.9%) disorders: 7,575 (1.1%), 10,956 (1.6%), 184,061 (27.5%), 173,365 (25.9%), 413,914 (61.9%). During 7.4±1.1 year median follow-up, 8,932 events occurred (1,235 7,697 without). Patients showed higher than (25.4/100,000 PY vs. 17.7/100,000 PY). After adjusting multivariate variables, still significantly (Hazard ratio (HR) 1.507, 95% confidence interval (CI) 1.418–1.601). When analyzed according each disorder, or had 2 times (HR 2.105, CI 1.461–3.031; HR 2.006, 1.307–3.079, respectively), depression, an approximately 50% 1.553, 1.397–1.727; 1.635, 1.47–1.818; 1.478, 1.372–1.592, respectively). Subgroup analyses generally consistent main result. Conclusions Young disorders, ie. insomnia, anxiety, AF. Awareness screening for these high-risk populations should considered.
